zap extension setup issue

    getting closer with forum help and documentation but i am stuck again. have 2 sip phones configured and talking. added zap ext. can dial and talk from sip phone to zap ext. at first i couldn't dial from zap ext to sip. removed then readded it using freepbx config. was then able to dial from zap to sip but a different ext (6001) which i did not setup shows as the ext id. also i get 2 cdr's for each answered call. below is the cdr and zapata config files. i setup ext 1002 and 1009. what am i doing wrong?
