Unable to use break out from queue - get sql error

    I have setup a simple IVR that directs all callers to a queue first and rings a phone(s) while playing on-hold music for 30 seconds, then goes to voicemail if no answer. During the 30 second wait I want to give callers the option to 'break out' of the queue and go to voice mail.

    I created the necessary IVR for the break out destination but when I try to modify the existing queue to use the break out (choose it in the drop down menu under periodic announcements) I get the following error:

    DB Error: constraint violation

    error adding to queues table

    I then have to go back to the original queue and re-add the static agent extension as it is magically removed after the error.

    Any ideas as to what is going on or how to fix this?
