Ah, indeed it was a context issue.
What I've been able to gather is this:
The extensions were added with no voicemail initially,
and when they were put in after the fact, they had the context
'device' (so 101@device in the mailbox entry)
When I created a new extension and set up voicemail at
creation time, it used '101@default' for the mailbox
entry, and it worked fine.
I don't know that it's a "bug," but it is at least an
