Problem with VPN connections and users Ipsecs

I ask if someone could help me with the following problem

I have to Elastick working in an office with local network 192.168.1.0 with a lynksys router (VR042) has a VPN connection with another office, the second has a network 10.0.0.1.
The lynksys provides your users see, PPTP and IPsec connection users and tunel vpn.

Everything works fine (connections between attachments, sending and receiving voice) network server 192 where the asterick, to look also works well between them and the users connected with pptp.

But I have problems with users of the second network (connected via VPN, and users ipsecs)
Both when they call or when they receive calls from users of the 192 or pptp users, they can listen to the other sip, but the sound of their softphone is not transmitted to other users.

Lei about changing values in sip.conf. uncomment the last line of sip.conf which enables sip_custom_general.conf include

cat sip_general_custom.conf
nat = yes
externip = 181.211.84.334
localnet = 192.168.1.0/255.255.255.0; 10.0.0.0/255.255.255.0 # # # add this but did not change anything
externrefresh = 10

cat sip_custom.conf
language = en
videosupport = yes
allow = ulaw
allow = g729
allow = G723
allow = H261
allow = h263
allow = h263 +
allow = h264
allow = h264 +

Clarified by the network router 192, the ports are forwadiados 5060 to 5070, and 10000 to 20 000 to the ip of the machine that has the elastick

At this, my question is

1) is well put sip_general_custom.conf values or must create the file sip.conf ?

2) Someone could tell me or give me a clue where the problem that the voice and / or media channels of the sip, non-network where the 192-asterick-(the sip that connect through the vpn ipsecs or user) your voice does not reach the other sip.

From already thank you very much and I apologize if they are already covered
Sorry for my English.
Regards

the " # " as the first character in asterisk conf files means include the following file, it does not comment out the line " ; " is the comment character.
