Intraoffice dialing not working

Customer can make and recieve calls via a PRI but when they transfer or call another extension it rings for a sec then hangs up, can anyone decipher this and tell me what could be wrong or where I should start. Elastix 1.3, PRI, Aastra IP phones.

Look like you re getting a databse error, do you have a custom setup?:
DBdel: family=BLKVM, key=4600/SIP/4603-0a023440

No just Elastix 1.3, Aastra XML scripts and a PRI. I am doing a re-install tomorrow morning but it sucks.
