Dialer Hang UP

I have the predicitve dialer running but I'm having problem using the form because I want to record a result for each successful call. I have the form created and it is working fine, but if the client or the agent Hangs up, the form goes away and my agent can't do anything on the form. Is there any configuration for after call work? Thanks!
